- Medicaid, on referral basis only
We are NOT providers for United Healthcare or Cigna.
If your insurance company is not listed we ARE NOT a provider. It is the patients responsibility for any additional copay's and deductibles that are applied to non-participating providers.
It is always best to call your insurance company to determine if we are participating providers.
